Single-floor office. Wheelchair accessible. Patients with significant vision impairment are welcome to bring a companion.
What to bring
Photo ID, insurance card, completed new patient forms, current medication list, sunglasses (you'll be dilated), and a driver — your vision will be blurred for several hours after dilation.
